Triages after-hours telephone calls from hospice patients, family and caregivers to respond to inquiries or concerns and to perform home visits as needed. The incumbent in this position must reside in our service area (counties served - Adams, Columbia, Dane, Dodge, Grant, Green, Green Lake, Iowa, Jefferson, Juneau, Lafayette, Marquette, Richland, Rock, Sauk, and Walworth).
Responds to incoming calls from our Call Center staff for after-hours hospice services in a timely manner. Calls the patient, family member and/or caregiver back to gather information in order to address the inquiry or concern. Exercises good nursing judgment in responding to patient /caregiver questions; advising on how to address patient symptoms and/or other concerns; providing re-education; assessing the patients condition; and identifying a course of action to ensure for patient quality of care. Ascertains the need to conduct a nursing visit to the patients residence. Communicates with physicians as needed to obtain orders to manage symptoms of hospice patients. Communicates with care team members as appropriate. Responds to the patients residence in the event of a patients death. Ensures the death summary is complete and accurate. Conducts timely and accurate documentation in the clinical record. Travels timely to patient homes and facilities to provide end of life skilled nursing care within the established hospice plan of care. Provides direct patient care including performance of tasks such as taking vitals, performing wound care, IV care, ostomy care, blood draws, injections, and the physical assessment of the overall patients medical condition. Reviews, sets-up, and administers medications as necessary per the established hospice plan of care. Conducts patient admissions and discharges as assigned. Ensures patients assessments and reassessments are completed in a timely and accurate manner. Works in collaboration with the hospice team members to assure coordination of care is provided to manage pain and ensure quality end-of-life care. Communicates with other members of the patients care team on the patients condition. Contacts and consults with the hospice team regarding the plan of care. Provides regular updates on the patients condition. Communicates significant changes in patient condition. Documents visit notes, treatments, and care. Records and reports symptoms and changes in patients condition. Verifies that physician orders are complete, accurate and cover the care and visits provided. Documents orders in consultation with the physician, advanced practice nurse practitioner, and interdisciplinary team. Ensures required visits are completed in a timely manner. Provides information, instruction and education to the patient, patient family members and caregivers in proper care. Verifies that instructions are being followed and carried-out. Completes and submits timesheets or time logs to supervisor on a timely basis which accurately document hours, mileage, travel and other reimbursable and non-reimbursable time worked. Attends and participates in mandatory meetings, in-services, and conferences. Regular and reliable attendance at work.
