The purpose of positions in this classification is to complete a comprehensive medical assessment to admit patients into home care and hospice.
Contacts patients to schedule admission visits. Travels to patient homes, CBRFs, assisted living facilities, hospitals, etc. to meet with patients and patient family members. Determines if patients meet home bound criteria and skilled nursing services for admission.
Completes admission paperwork and verifies insurance information. Completes a comprehensive medical assessment by entering data into the clinical computer program. Ensures that patient assessment is completed in a timely manner.
Provides information, instruction and education to the patient, patient family members and caregivers in proper care.
Contacts and consults with the physicians to notify and communicate significant needs at the start of care and to obtain orders. Documents any verbal orders for the start of care.
Communicates the patients plan of care for nursing, therapy, social services and other disciplines and services following the initial assessment with appropriate staff and the physician. Coordinates the transfer of the patient to the appropriate branch.
Completes and submits timesheets or time logs to supervisor on a timely basis which accurately document hours, mileage, travel and other time worked.
Attends safety training sessions and ensures for compliance with safety rules and procedures. Identifies and notifies supervisor and Safety Officer of potential safety hazards. Reports any work-related accidents and injuries in a timely manner. Complies with the procedures for documenting and submitting leave and light duty time.
Attends and participates in mandatory meetings, in-services, and conferences.
Assists with audits as requested.
Conducts Home Connections as assigned. Travels to patient homes and conducts a health coaching visit following a patients discharge. Regular and reliable attendance at work. MARGINAL JOB DUTIES
Sets up telehealth monitors.
MINIMUM EDUCATION AND EXPERIENCE Associates degree in Nursing with one to two years progressively responsible home care nursing experience that includes patient admissions experience and experience with IV and wound vac or any combination of education and experience that provides equivalent knowledge, skills or abilities. Prior acute care, triage, or other nursing experience may be considered in lieu of home care experience. Current State of Wisconsin RN licensure required.
SPECIAL REQUIREMENTS This position requires travel to patient homes and meeting locations. Must maintain a valid Wisconsin drivers license. Must maintain an acceptable drivers record per company policy. Must maintain proof of current auto insurance. Must have access to a vehicle in good working condition. Requires evening and weekends. Positions in this class may be required to work holiday and on-call. Requires conduct of 10 visits/week, 2.5 visits/day or 4 hours/visit (based on a 10 hour work day, 40 hour work week).
